FPA Members: You must be logged into the website to complete the entry form!

This will help keep track of multiple package entries and the entry fees. Otherwise, the non-member entry fee will appear during the entry submittal.

Entry Fees: For FPA members, the first package entry is $250. Additional packages may be entered for $150 per package entry. Non-members are permitted only one package entry, and the entry fee is $1,000.

In order to use the entry form below, please download a copy of the 2026 Official "Call for Entries." The Official "Call for Entries" provides an overview of the Achievement Awards competition, the rules including eligibility requirements, entry fees, and complete instructions on how to enter the competition.

The deadline for entries is October 23, 2025. Late entries will NOT be accepted.

This entry form outlines the key criteria attributes for evaluating expanding the use of flexible packaging, technical innovation, printing, shelf impact, packaging excellence, and sustainability categories. These attributes are what differentiates flexible packaging from other packaging formats to provide extraordinary packaging solutions.

Categories

Please indicate the attribute category(ies) you are entering by selecting one or more of the following categories and provide a description of how the attribute applies to the package. Use the description for each attribute category as a guide to develop your responses. Please note that a package may be entered in one or more of the six categories at no additional cost.

Entry Tip: Providing a detailed description for each attribute category you are entering your package in will increase the chances of winning an award.

Supporting Documentation

For each category, provide a description of how that category applies to the package. A detailed description will increase the chances of winning an award. You can also upload a supporting document for each category. Accepted file formats include PDF, Word Document, and Textile with a maximum size limit of 8MB.
